By booking I hereby give my consent to: The aesthetic medical treatment is known as Botulinum Toxin Injection (Botox). All aspects of tBotox treatment and potential risk and complications will be fully explained to me. I will have the opportunity to ask questions about Botox and the procedure, including the limitations. I will have the chance to discuss alternative forms of treatment if I am not a candidate of Botox. I understand that the practice of Botulinum Toxin/Botox is not an exact science, and therefore no guarantee can be given as to the results of the treatment. I accept and understand that the goal of the treatment is an improvement, not perfection. Botulinum toxin/Botox is for temporary improvement of wrinkles or expression lines caused by muscles’ action, specifically in the frontails, glabella, and lateral orbicularis region. Other areas may be discussed and treated appropriately. I understand that a small amount of purified Botulinum Toxin/Botox will be injected into my muscle, which will cause weakness and/or relaxation of the injected muscle, which will reduce movement of that muscle, causing a reduction of the skin wrinkles. I understand that the effect of Botox injections may take 3-14 days to appear. The duration of the effect varies from patient to patient. Treatments are recommended approximately every four months. My expectations of Botox are realistic, and I understand that the results are not guaranteed. For maximum results, more than one application may be required to continue treatment for desired effects. I will need to make a follow-up appointment in 2 weeks. I understand the potential side effects can occur despite the suitable election of the procedure and its correct application. I understand my provider will take every precaution to minimize or eliminate negative reactions such as mild tenderness, swelling, redness, pain, itching, bruising and tenderness at the injection site, headache, asymmetry, upper eyelid ptosis, eyebrow or eyelid ptosis, diplopia, and allergic reaction. I have been informed that it is essential to know and share my personal history to establish any possible allergies to medicine. Pre-Treatment Instructions for Botox: Discontinue Retin-A two (2) days before and two (2) days after treatment. Avoid blood thinners. Avoid alcoholic beverages at least 24 hours prior to treatment. Schedule 4 weeks prior to a special event. Reschedule if you are sick within twenty-four 24 hours prior to treatment.
